In healthcare, there is occasionally a gap that occurs between the expectation of how something is performed and the actuality. “Performance gaps have a variety of causes, including deficits in knowledge, lack of skill, or inability to transfer knowledge and skill to the practice environment” (Dickerson, 2017, pg. 95). In the acute care setting, it is the responsibility of the Nursing Professional Development (NPD) Practitioner to identify practice gaps and create solutions to remedy the problem.
The hospital selected to be analyzed for this discussion is a medium sized acute care facility located in the Inland Empire Region of Southern California. It contains a total of 238 beds: 160 acute care beds and 78 rehabilitation beds. The Emergency Department treats over 60,000 patients annually. This facility is accredited by the Joint Commission as a primary stroke center and for behavioral healthcare. The California Department of Health has granted licensure for percutaneous coronary intervention.
While completing the hospital comparison, it was discovered that this facility scored lower than the national and state average on educating patients about medication they are receiving while in the hospital. This hospital scored a 56% in this category, whereas the national and state averages were 66% and 63% respectively. This gap may have resulted from unclear expectations either by management or patients, and cannot simply be remedied by education alone (Dickerson, 2017). The NPD Practitioner may want to consider performing an analysis of the job, work setting, and learner to determine why nurses are not explaining the medication they are administering to patients. A job analysis takes a look and the how and what staff members do and how it affects the outcomes of their work (Sleezer, Russ-Eft, & Gupta, 2014). A work setting analysis examines how the workplace culture, mission, and values contribute towards staff’s performance, and a learner analysis determines if the teaching style is congruent with the learner’s needs (Dickerson, 2017). From this data a NPD Practitioner will be able to foresee the learning needs of staff and create a corrective plan of action to close the practice gap (Harper & Maloney, 2016).
Additional data regarding practice gaps can be collected from a variety of methods including surveying patient’s experiences and observing staff (Dickerson, 2017). Patients can complete surveys with opportunity available to provide comments on their level of expectation about the information they received regarding the administered medications. Staff can also be audited randomly to assess for barriers to informing patients about medications such as limited time, lack of confidence, or poor communication (Bowen, Rotz, Patterson, & Sen, 2017).
Determining practice gaps is an integral role of the NPD Practitioner. Analysis of the job, work setting, and learner will help the NPD to formulate corrective actions to close the gap. Solutions can be found by surveying patients and observing staff practices for compliance with expectations.

Gap Analysis
The purpose of this week’s discussion is to understand Gap analysis and how to go about assessing areas in need of improvement by doing a learning needs assessment on a hospital of choice and analyzing what practice gaps in learning are noted. When the learning need is chosen thorough analysis of statistics and using the Hospital Compare website, discussion on how to approach assessing the learning needs of the gap and what resources can be used will be examined.
Hospital Gap Analysis
A gap analysis is one of several means in which to conduct a learning needs assessment (LNA) (Pilcher, 2016). The gap analysis being conducted for this week’s assignment is on a local hospital which is a medium-sized acute care facility with 242 beds located at the Jersey shore. This is considered a small town hospital and is in a resort area that sees an increased influx of patients during the summer months, and drops off considerably in the fall and it is currently associated with the University of Pennsylvania hospital system. According to Pilcher (2016), other means by which to conduct an LNA include organizational needs, individual learner needs, job performance needs, and patient needs, noting that some areas can blend into each other. Methods to conduct research into LNA can include formal gap analysis through literature reviews, staff/patient interviews/ surveys, and risk assessments to name a few (Pilcher, 2016).
A gap analysis was conducted using the hospital compare website that provided the ability to not only view the statistics for the hospital of choice and compare to state and national averages, but to add other hospitals in the area and compare how they measure up to each other (U.S. Centers for Medicare and Medicaid Services. (n.d.). In this gap analysis, only one hospital was chosen and rated against state and national averages. The results overall noted comparable statistics across the board to the state and national averages, but one statistic stood out. In the survey of patient experiences or Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) the percentage for patient’s understanding of their care upon discharge was only 47%, but notable is that it is comparable to the state average of 48% and just under the national average of 53% (U.S. Centers for Medicare and Medicaid Services. (n.d.). This seems to be a deficit in health literacy that is in need of improvement, not only in this hospital but nationwide. According to a study done by Innis, Barnsley, Berta, & Daniel (2017) on healthcare literacy discharge practices, 36% of Americans and 60% of Canadians lack health literacy in understand discharge medications and instructions and needs. This is an area that seems to needs improvement and is an opportunity to provide education that will improve overall patient care.
Conclusion
Gap analysis provides an opportunity to evaluate areas that are in need of improvement. It provides the ability to find gaps in areas of healthcare where, through educational opportunities, improvements can be made and the overall quality of patient care can be improved. Using websites such as hospital compare and researching through search engines for peer-reviewed evidenced-based sources can provide the knowledge of where the gaps are and how to go about creating an educational design that will result in improvement in the gaps that were found. The end results in providing the most comprehensive and best quality care for the needs of the patients being served.
