3) Trainee Assessment form – develop a test to assess the knowledge, skill, or ability being trained. 1 page
4) Trainer Evaluation form – create an evaluation of you and the training for the trainees to complete. Typically these are Likert scales, 1 – 5 range, on the trainer’s presentation and content and its usefulness to their job. 1 page
